Please understand that most medical insurance plans do not cover 100% of the cost of treatment and that filing a claim does not relieve my responsibility for any charges whether or not reimbursed by insurance coverage.  Therefore, you are responsible for all charges incurred for treatment.

 

All insurance benefits payable on behalf of the patient shall be sent directly to this office.

 

Your insurance policy is a contract between you , your employer, and your insurance company.

 

We do not routinely accept Medicare assignment, but this can be evaluated by request. 

 

Please be aware that some, and perhaps all, of the services you receive may be non-covered or not considered reasonable or necessary by Medicare or other insurers.  It is your responsibility to know what is and is not covered by your insurance policy.  Non-covered services must be paid in full at the time of service.

 

If at any time your bill becomes a significant hardship, please discuss payment arrangements with the Practice Administrator.
