We participate in most insurance plans, including Medicare. If you are not insured, payment in full will be due at time of service including a 15% discount unless payment arrangements have been made prior to your appointment. If you are insured by a plan we participate with but do not have an up-to-date insurance card, payment in full for each visit is required until we can verify your insurance coverage. It is your responsibility to know and understand the coverage’s and deductible’s of your insurance plan and that insurance is only a method of reimbursing you for fees due and is NOT a substitute for payment. Please contact your insurance company with questions regarding your coverage. Co-payments and deductibles must be paid at the time of service. We accept all forms of payment including major credit cards. Co-payments are part of your contract with your insurance company. We must obtain a copy of your valid primary insurance card to provide proof of insurance and insurance billing information. If you fail to provide us with the correct insurance information in a timely manner, you may be responsible for the balance of a claim.If your insurance changes, please notify us before your next visit so we can update your account and help you receive your maximum benefits. Please bring your new card(s) with you to your next office visit, we will copy it and keep it in your electronic chart. If your insurance company does not pay your claim in 90 days the balance will automatically be billed to you.
